Adding PHQ-9 to Client Files
To add the PHQ-9, along with other built in surveys, please follow these steps:
- Right-mouse click on the file form and select ‘Customize Layout’.
2. From the Customization box,
a. Click on ‘Add’
b. Scroll down and then click on ‘Custom Form’
c. Click ‘OK’.
d. Drag drop and the ‘Custom Form’ item on to your client file form.
e. You can drop it into an existing tab grouping or, on to the main form where you will then create a 'Tabbed Group'
f. To create a ‘Create Tabbed Group’, right mouse click on the ‘Custom Form’ box
g. Click on ‘Create Tabbed Group’
h. Drag and drop the newly created tabbed group next to a tab on your form. When you see the box outline, release the mouse button to drop it in place.
i. Right mouse click on the tab and select ‘Rename’ the tab to a name of your
j. Click on the ‘X’ on the ‘Customization’ box when
3. To ensure that the addition is saved on the file, you will need to close out of the file. These steps will need to be followed for all of your File Types (EAP, SUP ).
4. Click on ‘Model File’ and then ‘Save / Distribute’ under ‘Tools’ to distribute the form changes to all users. You may need to ask a colleague with administrator rights to complete this task.
